MEDLink phone: 415-399-9769 Doctor’s office is located in the Historic Central Building, across from Oakland’s City Center, in front of a BART 12th Street exit. Parking: Paid parking is available at the Ken’s Garage at 420 13th St., between Franklin & Broadway. Parking rate is $4.0 per hours with a maximum of $16.00 for a whole day. (Please note: This garage cannot accommodate big cars such as SUV or trucks, only compact cars); Another parking garage is available at Oakland City Center garage located beneath City Center, accessed via entrance on 14th Street between 1300 Clay St and 505 14th St buildings, or via 11th St. just pass Clay Street. The parking rate will be $2.00 every 20 minutes, $6.00/hr., or $35.00 all day. Please be punctual for this exam as it may last from 1 to 2 hours. In the event that the doctor may be delayed by traffic conditions, there may be no receptionist to receive you at the office upon your arrival, we request that you wait for the doctor patiently or call MEDLink at 415-399-9769. If this is the 1st time you are seeing the doctor, please be sure to complete the doctor’s history questionnaire enclosed in FULL and bring the completed questionnaire along with you to the appointment. If this is a re-evaluation, please be sure to UPDATE your information (indicate changes in your symptoms, changes in your ability to perform activities listed in the questionnaire Page 3, changes in medical history, new operations since you last saw Dr. Nacouzi, changes in medications, any new traumas/injuries sustained since you law saw Dr. Nacouzi) bring the questionnaire along with you to the appointment. If there is no changes, indicate so on the questionnaire. If you misplaced or did not receive the history questionnaire, please call 415-399-9769 to request another copy.

Take I-80 West to Oakland Merge onto I-80 W Take the exit on the left toward Downtown/Oakland/Stockton Continue onto I-580 E Take the exit toward Downtown Oakland Continue onto I-980 W Take the 18th St exit toward 14th St. Merge onto Bush St. Turn left onto 14th St. Doctor’s office is on 14th Street and Broadway

Take I-880 North towards Oakland. Merge onto I-880 N Take the exit toward Broadway/Downtown Merge onto 6th Street Turn right onto Broadway Turn right onto 14th St. Doctor’s office is on 14th Street and Broadway
