If this is a reevaluation, please be sure to UPDATE your information (indicate changes in your symptoms, changes in your ability to perform activities listed in the questionnaire Page 3, changes in medical history, new operations since you last saw Dr. Nacouzi, changes in medications, any new traumas/injuries sustained since you last saw Dr. Nacouzi) bring the questionnaire along with you to the appointment. If there is no changes, indicate so on the questionnaire.
