Please Note: Please be punctual for this exam as it will last 1 hour. You will be seen by Dr. Roger Nacouzi in the medical office of Dr. James Soong. The doctor’s office is located on the second floor of the Saint Francis Hospital Medical Building/Pharmacy.

Parking: Paid parking is available in the Saint Francis Hospital Medical Building Parking Garage just before the Hyde Street Intersection. PLEASE NOTE: This parking garage will not allow trucks or vans due to the height of the garage, only cars are accepted. Parking is not validated, at $2.00 per half hour. Take the elevator to doctor’s office on the Second floor, Room 234.

If this is the 1st time you are seeing the doctor, please be sure to complete the doctor’s history questionnaire enclosed in FULL and bring the completed questionnaire along with you to the appointment.

If this is a re-evaluation, please be sure to UPDATE your information (indicate changes in your symptoms, changes in your ability to perform activities listed in the questionnaire Page 3, changes in medical history, new operations since you last saw Dr. Nacouzi, changes in medications, any new traumas/injuries sustained since you law saw Dr. Nacouzi) bring the questionnaire along with you to the appointment. If there is no changes, indicate so on the questionnaire.

From Peninsula / 101 North
Take Highway 101 North to San Francisco
Exit at 9th Street / Civic Center
Turn left from exit & follow 9th Street across Market Street
9th street then turns into Larkin
Turn right at Bush Street and proceed about one block
Turn left into the Saint Francis Hospital Medical Building Parking Garage just before Hyde Street intersection

From East Bay / Bay Bridge
Take Bay Bridge to San Francisco
Exit at 9th Street / Civic Center
Turn left from exit onto Harrison Street
Turn right on 9th street which then turns into Larkin
Turn right at Bush Street and proceed about one block
Turn left into the Saint Francis Hospital Medical Building Parking Garage just before Hyde Street intersection

From Golden Gate Bridge
Take Lombard Street toVan Ness Street
Make a right onto Van Ness Street
Follow Van Ness to Bush Street and make a left on Bush
Turn left into St. Francis Medical Office Building/Pharmacy Parking Garage
just before the Hyde Street intersection.

Take BART to San Francisco's Embarcadero Station
Take the escalators marked Drumm to the street level
Catch the #1 California bus west on Sacramento & Drumm
Exit at the Hyde / Sacramento bus stop
Turn left onto Hyde Street
Walk up Hyde two and a half blocks until you are between Pine & Bush Streets
Office is on the corner of Hyde and Bush, across from Saint Francis Memorial Hospital
